Ha! I'm amused as we spend hours doing the same thing but the other way round - making sure the output is A4! It's quite frequent that a desktop or network printer will demand that "letter" paper is loaded because something somewhere is not A4. (BTW, is Letter the same as 8½"×11"? I've no idea.)
It can be a mammoth job tracking it down - checking the system locale is correct (UK for us) and trawling through printer driver properties in the Control Panel and applications' print menus to ensure everything is A4.

And which version of Distiller?
I think the job options of later versions of Distiler have a default page size. If it's set to A4 (210mm×297mm), then this could be the cause. However the default setting is 8½"×11", not A4, and as Stan says Distiller usually accepts the page size defined in the PS file.
